This mod is a grenade… a nuke grenade :) it can be found after you exit vault 111, on the left side in the back of an apc,or it can be crafted at the chemical workbench under the grenade tab.
As always I built this mod with in game assets and I done it my self and like most of my mods I do not know if anyone has made one like it or not.
Please Note this is an old mod i made for xbox and pc i thought i re up-load it for ps4, it being so old i will not be updating it any. ( well i don’t plan on it anyways but you never know)
